Full Waiver: Christmas Break Soccer Camp Waiver & Release of Liability

1. Assumption of Risk

I, the undersigned parent or legal guardian, understand that participation in the Christmas Soccer Camp involves physical activity and the inherent risk of injury. These risks include, but are not limited to, falls, collisions, contact with other players, or environmental conditions. I acknowledge that these risks are inherent and cannot be completely eliminated even with proper supervision and training.

I voluntarily assume all risks associated with my child’s participation in this camp, including any injury, illness, or property damage that may occur before, during, or after camp sessions.

2. No Insurance Notice

I understand that this camp, its organizers, coaches, and staff do not carry liability or medical insurance for participants. I agree that I am solely responsible for any medical expenses or damages resulting from my child’s participation, and I certify that my child has personal health insurance or coverage sufficient for any injury that may occur.

3. Release and Hold Harmless Agreement

In consideration for my child’s participation, I hereby release, discharge, and agree to hold harmless Coach Jonny, Coach Pollo, MatchFit, their assistants, staff, volunteers, field owners, and all related personnel or affiliates from any and all claims, demands, or causes of action arising out of any injury, illness, or loss that may occur during or after the camp—whether caused by ordinary negligence or otherwise.

This release applies to any and all known or unknown claims and includes attorney fees and legal costs incurred in defense of any claim arising from my child’s participation.

4. Medical Authorization

In the event of a medical emergency, I authorize camp staff to obtain or administer medical care deemed necessary for my child, including transportation to a medical facility. I agree to be fully responsible for any related expenses.

5. Media Release

I grant permission for photos or videos of my child taken during camp to be used for promotional, educational, or social media purposes by the camp organizers.

6. Acknowledgment and Consent

I certify that I have read and fully understand this waiver, that I am the parent or legal guardian of the participant, and that I am voluntarily agreeing to these terms. I understand this document affects my legal rights and that by electronically acknowledging it, I waive the right to pursue claims against the camp organizers and staff.
